Description

Use the User page to view, create, or edit your account information. Some of the information in this page is set up by the Orchard System Administrator when your account is established. You may only modify the information as described below.

Access

You must have the necessary security right to access the User page.

  • Select the My Preferences menu button and then select the User submenu link.
  • Select the User submenu link on the Contact, Application, Order Choice Lists, Profile List, Result Delivery, or Associates setup pages.

Entering User Information

Last NameEnter the user's last name. This is a required field.
First NameEnter the user's first name.
Middle NameEnter the user's middle name.
DegreeEnter or modify your degree or professional credentials.
Full NameDisplays the user's full name.
Local NameEnter or modify the name that the program should display for you on reports in this field.
Time ZoneSelect your time zone from the drop-down list.
UPINEnter or modify your Medicare-provided Unique Provider Identification Number (UPIN) in the field.
NPIEnter or modify your National Provider Identifier (NPI) number in the field.
SSNEnter or modify your Social Security Number in the field.
Tax IDEnter or modify your business tax identification number in the field.
CLIA IDEnter or modify your CLIA identification number in the field.
SaveSelect to record your changes.
Discard ChangesSelect to clear your changes.


Avatar Options

Load AvatarSelect to upload a unique image from your local machine.
InitialsSelect to select as default avatar option.
UserSelect to select as default avatar option.
SuitSelect to select as default avatar option.
DoctorSelect to select as default avatar option.
NinjaSelect to select as default avatar option.
AstronautSelect to select as default avatar option.
SaveSelect to save the changes.


Reviewing Practice Memberships

Member of These Practices

The Member of These Practices section of the page lists the practices with which you are associated.

Note

You must contact your Orchard System Administrator if you need to modify the associated practices.

PracticeThe list includes the name of the Practice. For providers, the list also includes the lab accounts related to the providers' practices.
Lab AccountsThe column lists all of the lab accounts that have a defined account number in the Lab Accounts page. If there is no defined lab account, the column contains a link to the Lab Accounts page where you may define the account. Select the name of a lab account in the list to open the Lab Accounts page, where you may view or modify the lab account details for the selected practice, including the account number.
