Additional Info Fields
| Acquire Images | Click this button to open the Set TWAIN Device pop-up. Select the TWAIN device to use by clicking the button with the name of the device. Pop-up options:

| Switch to browser upload | Click the Switch to browser upload link to upload an image from the computer via the Choose File button. |
| Switch to TWAIN application | Click the Switch to TWAIN application link to view the Acquire Images button. |
| Remove All | Click this button to remove all images from the table. |
| Remove Selected | Click this button to remove only the selected images from the table. |
| Convert Selected into Single PDF | Click this button to convert the selected images into a single PDF. |
| Select | Select this checkbox to select images from the table. |
| Arrows | Click the up and down arrow buttons to sort the images. |
| X | Click this button to delete the images from the table. |
| Host: Date | Enter a host date in the format MM/DD/YYYY. Select a date from the calendar view by clicking the calendar icon located next to the date fields. Click Now to automatically fill the fields with the current date. Click Clear to clear the fields. |
| Host: Date & Time | Enter a host date and time in the format MM/DD/YYYY HH:MM AM/PM. Select a date from the calendar view by clicking the calendar icon located next to the date fields. Click Now to automatically fill the fields with the current date and time. Click Clear to clear the fields. |
| Host: Free Form | Enter text. This customizable field can be used to record any type of information that requires a text field. |
| Host: Multi-select | Select a checkbox for your host address: Avenue, Boulevard, Drive, Lane, Street, or Way. |
| Host: Number | Enter a host number. |
| Host: Provider Search | Click in or enter text in to the search-as-you-type field to search for a host provider. Click the blue link located in the Name column to add the provider to the field, or click the magnifying glass icon to manually search for a provider. Click X to remove the provider from the field. |
| Host: Sample Type | Click in or enter text in to the search-as-you-type field to search for a host sample type. Click the blue link located in the Name column to add the sample type to the field, or click the magnifying glass icon to manually search for a sample type. Click X to remove the sample type from the field. |
| Host: User Search | Click in or enter text in to the search-as-you-type field to search for a host user. Click the blue link located in the Name column to add the user to the field, or click the magnifying glass icon to manually search for a user. Click X to remove the user from the field. |
| OK | Click this button to add the images to the page. |
| Cancel | Click this button to close the pop-up without saving any changes. |
